I'm also worked on same requirement recently and I solved it.
First of all, There is no option to assign smart form in FBZP.
What I did is, simply assigned the driver program in FBZP and in driver program I have added one parameter for smart form name or hard code if you want, because we are calling this program in F110 where we defines the variant.
Simply assign smart form name in variant or hard coded.
